If you can't figure it out from googling, you can just leave the fan disconnected initially and run the "pinout" command provided with the Raspbian OS; it displays a nice little diagram of pin locations.
(Update: On third look I did find a small diagram of the fan wiring in one of the included documentation cards, but a larger or more obvious one would have been helpful.)
The included fan is way, way too loud, I mean ridiculously loud. Using this as a media player, it can't be used.
Thus I bought a Noctua NF-A4x20 5V PWM, Premium Quiet Fan 40x20mm.
It is only 14.9 dB. I did have a 80mm USB fan that claimed to be 23 dB but even that is loud.
Yes the Noctua fan is too big for the case, I cut the case put the fan in, right above the heat sync, and literally hot glued it in
The Noctua fan does include a adapter to go to power and ground. Cut the connections of the included fan, strip all 4 wires solder the wires and wrap in electrical tape.
Other notes, the HDMI closest to the power is the one to use.
Without a fan cpu was 145 F with a fan cpu is around 100 F at idle .
My unit is also hooked up with a 4k tv but I had trouble getting smooth full screen video playback. I had to reduce the resolution to 1080 to get smooth playback.
The case and heat sinks for this specific package works really well and I haven't noticed any excessive heat or performance issues with it.
