The only improvement I would make on the product is to shape the nose air part a little different. The current shape is a bit clunky.
But if you have problems with your Eustachian tubes like I do (glue ear) then this exerciser is worth every cent.
For me it wasn't a quick fix but over a period of days I have noticed far less pressure in my left ear which has been wonderful. This is a long term solution for me. One that antibiotics could not fix. Remember to use it with a big mouthful of water. The swallowing is when the tubes open.
I am very happy that I bought it.
Just have your two AA batteries on hand to start using it when it arrives.

Bottom line: When correctly used for the purpose claimed (and explained very clearly by the manufacturer) this device works exactly as advertised.
Disclaimer: Eustachi will not cure nerve deafness, fix a perforated eardrum, solve your Meniere Syndrome, cause your eyesight to get better or your sex life to improve. However, if you have substantial eustachian tube dysfunction or blockage not accompanied by infection, this is a well-built and very useful tool to have in your treatment and prevention arsenal.
Principle: the principle of operation is quite simple. The Eustachi is simply a small battery powered pulse air pump. It is equipped with a nipple like orifice which fits to your nostril. When you close off the other nostril and swallow, the air pressure in your sinus cavity is momentarily and modestly raised. This process forces air through your Eustachian tubes. This causes them to stretch and expand and exercises the tiny valve which is located at the end of the eustachian tube just as it enters into the middle ear. In the process, it forces a small amount of air into your middle ear, which you perceive as "popping". This pop is caused by rapid pressure differences between the external atmosphere and your middle ear, which are separated from each other by your eardrum. Google a detailed phisological diagram of your ear, INCLUDING your Eustachian tubes. What is happening will be immediately obvious to any reasonably intelligent person when one examines a pictorial representation of the physical structure. Google is your friend.
A: The angle at which you couple the device to your nose is critical. If you get the angle wrong, the opening on the nipple will be blocked and no air will enter your nose. You MUST experiment until you find the right angle. You will know when you do because you will feel the pressure rise in your sinus cavity and in a few seconds your ears will "pop". I expect that the occasional "it doesn't work" critical reviews elsewhere on Amazon were the result of this mistake by the user. Some people are just chronically impatient and non-adaptive.
B. If you have a mustache or nose hair, they can interfere with proper sealing of the nipple surface to your nostril. This will result in air leaks which will prevent the Eustachi from working. Take the obvious steps to improve the sealing.
C. If you push the device too hard into your nostril, your nose tissue will simply collapse and thus totally block the nostril. The Eustachi cannot pump air through a blocked nostril passage. Ease up...you are making a seal, not delivering a Mike Tyson punch to the nose.
D. IMPORTANT: Once the pressure rises, which takes a few seconds, YOU MUST SWALLOW TO INITIATE THE EUSTACHIAN TUBE REFLEX. To aid this, definitely get a sip of water in your mouth and hold it until the pressure rises, then swallow.
E. Did you catch that? "Once the pressure rises, WHICH TAKES A FEW SECONDS" Yes, Eustachi is a small battery powered pulse air pump. Key words "small" and "battery powered"! Bottom line here is give it a chance. IMO the instructions are deficient in this regard. They suggest 2-3 second applications. IME this is insufficient for my personal sinus volume - but your mileage may vary. I suspect the manufacturer is being cautious so that some litigious person doesn't deliberately blow out an eardrum and sue them out of existence. Sad but probably true. In my opinion a better approach is to apply it until you actually feel the pressure begin to rise then swallow and you will hear your eustachian tubes open. It is possible that one side my open first. If it doesn't seem to open, switch nostrils and try again.
F. The manufacturer very clearly explains that this is a tool to exercise your eustachian tubes. Keyword here exercise. That means you have to do it more than once just like working out. Don't wait until your ears are all screwed up to begin using it. Use it prophylactically - multiple times a day. You will notice gradual improvement over several days. If like me you have terrible eustachian tube functionality you will notice improvements - but only if used properly and repeatedly.
G: if you know you have bad eustachian tube function, and you know we were going to fly, use common sense. 1. Exercise your Eustachian tubes multiple days and multiple times in advance of your flight, and 2. bring Eustachi with you on the airplane. In my case I have been virtually grounded for a decade because I simply could not tolerate the air pressure changes when flying. I am a recording engineer by profession and I have million dollar ears. I also have terrible eustachian tube function. This was a debilitating combination for me until I discovered Eustachi.
H. Some people have said "Forget this device, you can just pinch your nose and blow and do the same thing". They are well meaning but they are wrong. What they are describing is called the Valsalva maneuver. Scuba divers know it well. So do I, and I can tell you that when properly used, Eustachi will consistently outperform a common Valsalva maneuver. Why? One simple reason. When you "pinch and blow" the muscles in your sinuses and head tense up. Try it and pay attention. You will feel it happen. This tensing up squeezes off your Eustachian tubes. It is a built-in reflex you cannot overcome. For people with normal eustachian tube function, this is no big deal the tensing does not totally shut off their Eustachian tubes. In my case, because of my poor Eustachian tube function I could pinch and blow until I burst blood vessels and popped my eyes out of my head and the Valsalva maneuver simply will not correct the pressure differential. When using Eustachi, your head is relaxed and the pressure rises because the pump is forcing air into your sinus cavity. The difference between Eustachi and a valsalva maneuver is obvious and profound, IF you have poor Eustachian tube function.
Conclusion. This device works and works exactly as advertised. Build quality is sufficiently robust, typical overseas "A quality" injection molding, typical tiny print instructions that the mfg. could and should improve. Looks like it will last for years if treated with reasonable care. Take the batteries out if you are going to stick it in a drawer for three or four years.

Works in an unexpected manner and definitely recommend to anyone dealing with long-term issues involving the Eustachian tube. I personally did not get the immediate relief others seem to have gotten, but I am hopeful over the longer term that I can condition the tube back to some semblance of normalcy and restore hearing (or at least get rid of the clicking).
Update 2017.06.08: I'm bumping my rating to five stars. I now don't care what it costs, but this little device has really started to make some changes. I kept at it for the past week, using about twice a day, three or four times between both nostrils after pressurizing both ears quite hard a few times. I couldn't tell it was really doing anything, but finally, today I experienced a "purge" of sorts that tells me I have had an infection trapped in my Eustachian tube for years. Plus, during this time there was a sensation of pressure or something lodged exactly in the area where my left adenoid used to be, so it is undeniable this device had everything to do with it. Yes, it is disgusting, but for anyone that is dealing with something similar, you need to try this device.
